Coaching Agreement

This agreement is entered into by and between the Ultimate Lifecare Coaching ULC (hereby referred to as “Coach”) and any individual (hereby referred to as “Client”), whereby the Coach agrees to provide Coaching Services to the Client. This coaching agreement is valid as of the date of the initial booking via online scheduling services, and a signature or online signature is required during online confirmation of each purchased Coaching Service.

Coaching Services:

1.1 The Coach agrees to provide coaching services to the Client in accordance with the terms outlined in this Agreement.

1.2 Coaching sessions will be conducted virtually via video conference call, either using Zoom or Google.

1.3 The Coach will provide support, guidance, and accountability to assist the Client in achieving their personal and/or professional goals.

Responsibilities:

2.1 Client Responsibilities:

- The Client agrees to actively participate in coaching sessions and complete any action steps agreed upon during sessions.

- The Client acknowledges that coaching is not therapy or counseling and will seek professional help if needed.

- The Client agrees to be open, honest, and receptive to feedback provided by the Coach.

2.2 Coach Responsibilities:

- The Coach agrees to maintain confidentiality regarding all information disclosed by the Client during coaching sessions, except as required by law.

- The Coach will provide a supportive and non-judgmental environment for the Client to explore their goals and challenges.

- The Coach will adhere to professional standards and ethics as outlined by relevant coaching organizations.

Session Schedule:

3.1 Coaching sessions will be scheduled at mutually agreed-upon times between the Coach and the Client.

3.2 Sessions typically last for 60 mins and will occur once a week, unless otherwise agreed upon.

 

Fees and Payment:

4.1 The Client agrees to pay the Coach the agreed-upon fee for coaching services.

4.2 Payment terms and schedule will be outlined separately in an invoice or payment agreement.

4.3 In the event of missed or canceled sessions by the Client without 24 hours' notice, the Client may be charged for the session.

Intellectual Property:

5.1 All material related to the Ultimate Lifecare Coaching is subject to copyright and other intellectual property rights.

5.2 All materials, programming, and content may not be recorded, used, or reproduced without written permission of the owner.

Indemnification:

6.1 The Client should seek independent professional advice before undertaking any physical, financial, or medical actions as discussed during Coaching Services and inspired or motivated by Coaching Services or coaching relationship.

Termination:

7.1 Either party may terminate this Agreement with 20 days' written notice.

7.2 If the Client terminates the Agreement, any prepaid fees may be refunded at the discretion of the Coach, minus any fees for completed sessions.

7.3 The Coach reserves the right to terminate the Agreement immediately if the Client engages in behavior that is unethical, illegal, or disruptive to the coaching process.

Confidentiality:

8.1 Both the Coach and the Client agree to maintain confidentiality regarding all information shared during coaching sessions, except as required by law.

8.2 The Coach may only disclose information with the Client's consent or when necessary to protect the Client or others from harm.

 

 

By signing below, the parties acknowledge that they have read, understood, and agreed to the terms and conditions outlined in this Coaching Agreement.
